Activities Forms
Activities at North Pole Middle School include any extracurricular activities, including athletics, clubs and socials.
For students who live on the Eielson Air Force Base, a bus leaves NPM at 4:45pm and drops off at the Youth Center every day!
To participate in sports, students must complete the following forms:
Sports physical Concussion awareness Activity ConsEnt Form
If you are homeschooled or enrolled in a district school AND North Pole Middle School is your home attendance school, the following form is also required. Please note that students enrolled in a non-district and accredited online school, must be enrolled in 4 courses each semester at North Pole Middle School.
All activity forms are due the school day BEFORE the season starts!
Activity forms must be submitted through the link below. North Pole Middle is not accepting hard copies.
*Concussion Awareness and Activity Consent Forms can be filled out through the Google Form or the documents can be opened, filled out, and then uploaded in the Google Form.
